Rome Travel Study Application Form 2010

Once your application has been submitted you will be notified by e-mail confirming receipt of your application and given instructions on how to make your $300 deposit. You will not be registered into the program of your choice until your $300 deposit is received.

Applications will continue to be accepted after the program specific registration deadline. However, to ensure your spot, please apply today and make your $300 registration deposit. Early registration is recommended as enrollment is limited.

Refund of the deposit (less a $150 administration fee) will be made if a cancellation and refund form is completed and sent prior to February 19, 2010 for Intersession Programs. After that date, no refund may be possible.

Costs are based on a minimum number of participants. In the event that this minimum is not achieved, the result may be the cancellation of this Intersession. At that time you will be informed and your deposit will be returned in full.

Participants have the choice of taking the courses for either credit or audit (a student wishing to attend classes in a given degree credit course out of interest without being assigned a grade). Participants are paying a program fee for the cost of the program and are not paying for tuition separately so there is no difference in pricing for those that choose to audit their courses.

Section II: Accommodation

UNB reserves the right to place students in rooms at our discretion. We will try our best to accommodate participant requests but it is not guaranteed that you will be able to room with a preferred person(s) due to registration numbers and accommodation availability.(please see accommodation policy for further information)

To complete your registration you will be REQUIRED to submit the following documents:

  • A copy of your flight itinerary. This can be mailed or emailed to
    travelstudy@unb.ca
  • A photocopy of your passport and/or Visa is required 45 days
    prior to the commencement of your program. This may be sent by mail or
    dropped off to our office. If you do not already own a passport,
    you should now be applying for one.

  • A photocopy of your International Medical Insurance
  • A copy of your Student Loans Needs Assessment Form
    (necessary only if you have applied for a student loan to support
    your payment)
  • A photocopy of your signed UNB Tuition Waiver form
    (Required only if you are a UNB employee, dependant up to the
    age 26 and or spouse).
  • Signed UNB Liability Waiver
  • Signed UNB Medical Insurance Waiver
  • If attending from another University and you would like to
    transfer course credits you will be required to submit a copy of
    your Letter of Permission.

The University of New Brunswick is committed to protecting the personal information of our participants. The information collected on this application form and any documents submitted to the program will be used solely by the College of Extended Learning and the program directors to administer the program and destroyed thereafter.

Student Agreement

I understand that, as a student in the UNB Travel Study Program, I must:

  1. Follow the University’s General Regulations on Code of Conduct
    and of the Student Disciplinary Code. These documents can be
    found on page 28 of the calendar or online here and here and
    here . The Code applies to all students whether on a UNB
    campus or at an off-campus location that is the site of UNB
    programs. It extends, moreover, to all off-campus events to
    with which the University is associated.
  2. Assume full legal and financial responsibility for my stay abroad.
  3. Take full responsibility for program fees (even if I am a financial
    aid student); pay all program fees by the necessary deadlines;
    and read and understand the Travel Study cancellation policy
    and waivers.
  4. Make my own transportation and passport arrangements.
  5. Submit a copy of the front page of my passport.
  6. Understand that my participation in my Travel Study Program
    is voluntary and that Travel Study Programs carry with them
    unanticipated and unforeseeable risks of danger to my health
    and safety which may not be found in study on campus and I
    expressly agree to assume the risks by my voluntary
    participation in this non-required activity.
  7. Be responsible for all expenses related to personal accident,
    injury or illness which may occur during the time in which
    I am participating in the UNB Travel Study Program and be
    prepared to pay for all medical expenses in full at the time of
    treatment.
  8. Bear financial responsibility for single room accommodations
    based on medical necessity as determined by the Program
    Director.
  9. Grant UNB, its faculty and administrators, full authority to act in
    an attempt to safeguard and preserve my health and safety
    during my participation in the Travel Study Program, including
    authorizing routine or emergency medical treatment on my
    behalf and at my expense, and returning me to Canada at my
    own expense (if necessary).
  10. Respect the laws and customs of the host country, the UNB
    Student Code of Conduct, and all other reasonable standards of
    behavior during your Travel Study Program.
  11. I understand it is the sole discretion of the Faculty Program
    Director, with appropriate consultation from the
    administration of UNB Travel Study Programs to terminate
    the participation of any student whose conduct is
    determined to jeopardize the order, function, health or
    safety of program activities or its participants. There will
    be no refund of fees and the participant will be fully responsible
    for all transportation costs accrued while returning home.
  12. Forfeit any refund if I decide for any reason to terminate my
    participation in the program after it has begun.
